Chronic Care Management (CCM)

We are not focused on a single illness or condition.  The m.Care platform can be configured to monitor any patient population.  Our monitoring plans allow clinicians to customize the plans to their unique protocols and workflows.  We have standard monitoring plans for:

Transition Care Management (TCM)

Transitional Care Management (TCM) services address the hand-off period between the inpatient and community setting. After a hospitalization or other inpatient facility stay (e.g., in a skilled nursing facility), the patient may be dealing with a medical crisis, new diagnosis, or change in medication therapy. 

Transitional care management ensures patients who have a high-risk medical condition will receive the care they need immediately after discharge from a hospital or other facility.  The m.Care platform provides transition care management, with special attention to the 30-day window post discharge

Pre- and Post- Procedure Management

The m.Care platform supports the workflow around procedure date.  Patient monitoring can begin before a procedure and can include measurement readings, questionnaires, education material, etc.  Post procedure management is the more traditional remote patient monitoring.  Areas of focus include:

Maternal Health

The United States has a very high maternal death rate in comparison to countries with the same medical advancements and level of care.  In addition, Postpartum depression affects roughly 15% of women after childbirth or around 700,000 million women annually. The m.Care platform provides a robust solution for helping to address this crisis. We offer Monitoring Plans for:

  • High Risk Pregnancies
  • Post Discharge Care (mom and newborn)
  • Postpartum 

Monitoring plans include date-driven educational materials and text bank messages, which are delivered along the pregnancy timeline. We also offer fetal doppler integration, which helps support the remote patient monitoring model.
