The patient-facing mobile app is a set of patient-centered applets designed to be easily used by at-home patients to provide personal health information and manage their health care. 

The Care Management Portal is where the Care Team has access to patient dashboards, and where they create or customize patient monitoring plans for physical or behavioral health issues.

The m.Care system captures the information needed to bill RPM and CCM reimbursable CPT codes, and provides reports and reminders throughout the billing cycle to maximize reimbursement.
