"The LST/m.Care team is great to work with – they are quite knowledgeable in RPM processes, innovative and very responsive. We have been able to reduce our 30-day All-Cause Readmission rate for high risk patients discharging from the hospital from 20% to just single digits in less than a year of utilizing their software. It has been a great partnership so far."
